Join our dedicated team in the Leaving Care, Children’s & Education Services in Manchester, where you will play a crucial role in supporting young people transitioning to adulthood. This role is ideal for someone passionate about making a significant impact in the lives of vulnerable residents, ensuring they are prepared and supported to live independently.
Day-to-day of the role:
- Manage a caseload and maintain accurate case records, undertaking statutory recordings.
- Contribute to Pathway Plan Assessment and provide ongoing assessment to inform Pathway Planning.
- Undertake assessments for young people resuming education/training or presenting with other support needs post 21 years.
- Promote welfare and ensure the views and wishes of young people are considered in assessments, Pathway Plans, and reviews.
- Provide advice, practical assistance, and support to young people to enable successful transitions.
- Coordinate the provision of services and ensure young people make use of such services.
- Identify and supervise the delivery of support packages for young people living in the community.
- Ensure young people receive appropriate financial support and help them develop financial management skills.
- Advise and support young people in gaining access to appropriate accommodation.
- Work in partnership with a range of agencies to ensure an integrated, holistic response to meet the needs of young people.
Required Skills & Qualifications:
- Strong interpersonal skills with the ability to understand and communicate effectively with others.
- Analytical skills to engage with stakeholders and identify information needs.
- Excellent communication skills, both written and verbal.
- Proficient in planning and organising multiple tasks effectively.
- Problem-solving and decision-making abilities.
- Competent administrative and IT skills to maintain effective systems.
- Understanding of relevant legislation and statutory guidance including the Children Act 1989, Children (Leaving Care) Act 2000, and related regulations.
- Willingness to undergo a DBS check.
- Full driving license and access to a vehicle daily.
- Flexible to work between 08:00 – 20:00, Monday to Friday.
