Teqniq is searching for a Short Breaks Assessment Officer to work in the public sector.

37 hours per week.

3 month contract.

09:00-17:00

Job Description:

Enabling access via assessment of need, to Short Breaks Services for disabled children and young people.

Processing applications made directly by families/professionals and de-escalations from the Children's Disability Service.

Screening for eligibility criteria.

Assessing families needs through information provided, professional supporting letters, liaison with schools and other agencies and the Resource Allocation System (RAS). This is including the completion of home visits, and other visits necessary to gain a holistic view of the child/young person, capturing their voice/wishes – using observations and communication tools as necessary.

Identifying level of support needed and indicative budget.

Completing annual reviews for support packages and processing re-applications, to ensure services are still appropriate and meeting needs.

Following up and answering short breaks enquiries.

Assisting and enabling citizens to use their Personal Budget to meet their social care and or health care needs, by supporting them to plan and source both paid support, and unpaid informal support from their social networks and communities.

Completing person centred support plans and other mandatory documentation to request and refer to the appropriate identified services in line with the child/young persons identified SEND outcomes, and presenting the request at the Short Breaks Panel.

Workers will be provided with consistent supervision and support, whilst working flexibly within in a hybrid work model.
