What you will do:
- Manage a caseload of young people aged 17–25, undertaking Care Act assessments, reviews and support planning
- Apply the 3 Conversations Model to deliver person-centred, strengths-based interventions focused on independence and resilience
- Complete mental capacity assessments and contribute to Best Interest decisions in line with the Mental Capacity Act
- Develop and commission care and support packages, including personal budgets and direct payments
- Lead and participate in multi-agency meetings, including EHCP reviews, Section 117 aftercare planning, and Continuing Healthcare processes
Person Specification:
- Degree or equivalent in Social Work (BA/BSc/MA, DipSW or CQSW)
- Registered with Social Work England
- Strong working knowledge of the Care Act 2014
- Strong working knowledge of the Mental Capacity Act 2005
- Strong working knowledge of the Equality Act 2010
- Understanding of Safeguarding Adults procedures and statutory guidance
- Familiarity with the Preparing for Adulthood framework
- Familiarity with SEND processes
