About the Role:
We are currently recruiting for an experienced Outreach Worker to join a dynamic outreach service covering the Derbyshire and High Peak area. This is a fully community-based role with no fixed office location, working across GP surgeries, community centers, and alongside outreach teams.
Due to the nature of the role and the rural location, candidates must be a driver and comfortable travelling regularly throughout the area.
Key Responsibilities:
- Managing a caseload of 40+ service users
- Delivering structured key working sessions and recovery plans
- Supporting individuals aged 18+ with substance misuse issues
- Conducting assessments, risk management, and care planning
- Working collaboratively with outreach teams and partner agencies
- Providing harm reduction advice and promoting recovery pathways
Requirements:
- Proven experience in substance misuse / recovery services
- Ability to manage a busy caseload effectively
- Strong communication and engagement skills
- Full UK driving license and access to a vehicle (essential)
- Experience working within community or outreach settings is desirable
